Check boxes are added to the form when you want the site visitor to select one or more options. When the form is submitted, if the check box has been selected, its internal name and value are included in the results. Otherwise, only the internal name is included and the value is blank. Push button can be added to a form and by writing script action can be specified. You can modify the HTML code to include a script or use the script wizard to create a script.

From within the form select the Insert menu.

Select Form.

Click Push button.

The push button is placed on the form. Double click the push button to view its properties.
