Two radio buttons are created with the same name but different values to allow the user to choose between the two values.

Two check boxes are created with the same name but different values to allow the user to choose between the two values. Check boxes allow multiple selections.

The Submit and Reset buttons are the default buttons of the form which allows the user to either submit the form or to reset the values specified.

The browser now displays the FORM with the Text, Image, Check box, Radio button, Submit and Reset buttons.
