1. Insert a new layer (Choose Insert > Layer) above the layer named as 'Name Bg'

2. Rename the layer as 'Name Textfield'

3. On the layer named as 'Name Textfield', create an input textfield with the Text Tool , it should be approximately of the same height and width as of 'Name bg rectangle' and in the Properties panel apply the following text settings : Text Type = Input Text, Font = Arial, Font Size = 12, Text (fill) Color = #697A94, Text Align = Align Left, Line Type = Single Line, Variable name as Var = name, X position = 110 px and Y position = 29 px.

4. Insert a new layer (Choose Insert > Layer) above the layer named as 'E-Mail Bg'

5. Rename the layer as 'E-Mail Textfield'

6. On the layer named as 'E-Mail Textfield', create an input textfield with the Text Tool , it should be approximately of the same height and width as of 'Name bg rectangle' and in the Properties panel apply the following text settings : Text Type = Input Text, Font = Arial, Font Size = 12, Text (fill) Color = #697A94, Text Align = Align Left, Line Type = Single Line, Variable name as Var = email, X position = 110 px and Y position = 59 px.

7. Insert a new layer (Choose Insert > Layer) above the layer named as 'Suggestion Bg'

8. Rename the layer as 'Suggestion Textfield'

9. On the layer named as 'Suggestion Textfield', create an input textfield with the Text Tool , it should be approximately of the same height and width as of 'Name bg rectangle' and in the Properties panel apply the following text settings : Text Type = Input Text, Font = Arial, Font Size = 12, Text (fill) Color = #697A94, Text Align = Align Left , Line Type = Single Line, Variable name as Var = suggestion, X position = 110 px and Y position = 89 px.

1. Insert a new layer (Choose Insert > Layer) above the layer named as 'Name Textfield'

2. Rename the layer as 'Validation Mark 1'

3. On the layer named as 'Validation Mark 1', type ' * ' mark with the Text Tool and in the Properties panel apply the following text settings : Text Type = Static Text, Font = Arial, Font Size = 24, Text (fill) Color = #FF0000, Text Align = Align Left and Character Spacing = 0.

4. On the layer named as 'Validation Mark 1', select the ' * ' mark text, convert it into a Movie Clip (Choose Insert > Convert to Symbol) with the Center Registration point selected and name it as 'validation mark'

5. Select 'validation mark' movie clip at frame 1 of layer named as 'Validation Mark 1'. In the Properties panel give an Instance name to 'validation mark' movie clip as 'namemark' and set X position = 386 px and Y position = 25 px.

6. Select 'validation mark' movie clip at frame 1 of layer named as 'Validation Mark 1' and Choose Window > Actions.

7. In the Movie Clip Actions panel type the following ActionScript as given in the below image,

8. Insert a new layer (Choose Insert > Layer) above the layer named as 'Name Textfield'

9. Rename the layer as 'Validation Mark 2'

10. Select frame 1 on the layer named as 'Validation Mark 1', Choose Edit > Copy

11. Select frame 1 on the layer named as 'Validation Mark 2', Choose Edit > Paste

12. Select 'validation mark' movie clip at frame 1 of layer named as 'Validation Mark 2'. In the Properties panel give an Instance name to 'validation mark' movie clip as 'emailmark' and set X position = 386 px and Y position = 55 px.

13. Insert a new layer (Choose Insert > Layer) above the layer named as 'Name Textfield'

14. Rename the layer as 'Validation Mark 3'

15. Select frame 1 on the layer named as 'Validation Mark 2', Choose Edit > Copy

16. Select frame 1 on the layer named as 'Validation Mark 3', Choose Edit > Paste

17. Select 'validation mark' movie clip at frame 1 of layer named as 'Validation Mark 3'. In the Properties panel give an Instance name to 'validation mark' movie clip as 'suggestionmark' and set X position = 386 px and Y position = 85 px.
